- Physical Form – found here and on the TCA athletics resource board. You will upload a PDF of the physical form directly to RankOne. Click here for instructions.
- Medical History
- Student Profile – you will need to have your health insurance information on hand
- TCA Signature Page

here. We anticipate completing these requirements will take approximately 10 minutes.
In addition to the above, US athletes and their parents must complete the following documents in the
TAPPS Management System (“TMS”) app:
